How to Obtain a Copy of a Birth, Marriage, or Death Certificate

Certified copies of birth, marriage, and death records are available from the Town Clerk's Office. Copies of vital records may be obtained in person or by mail.

By Mail - Provide the name of the subject(s) of the record and the date of the event and mail your request to:

Enclose a business-size self-addressed stamped envelope for records to be mailed to you.

Note that only certified copies of records are issued by the Town of Milton regardless of intended use. Make your check or money order payable to "The Town of Milton".

Birth Certificates

This office will have birth records of anyone who was born in the town of Milton or whose parents lived in Milton at the time of the birth. A long-form birth certificate is available for $8.

Marriage Certificates

This Office will only have marriage records of any couple who filed their marriage intentions in Milton. We do not have the marriage certificates for couples that lived in Milton at the time of the marriage or couples whose marriage ceremony took place in Milton unless they filed the original marriage intention in our office. A certified copy of a marriage certificate is available for $8.

Death Certificates

This Office will have death records of any death that occurred in Milton and copies of records of Milton residents whose death has occurred within the State of Massachusetts. The fee for a certified copy of a death certificate is $8.
