Sub-metering of water and sewer certification form 

• Above form is the document needed if you are a landlord renting a property where occupants are responsible for paying their own water/sewer bill. 
  • Landlord is responsible for hiring a licensed plumber to ensure proper installation of water flow equipment. 
  • Plumber should fill out above form and return it back to the health department for documentation.
