The deadline to opt-out for FY2024 has passed. To opt out for the July 1, 2024 through June 30, 2025 billing period, please print and complete the form below. Tape the completed form to the inside of the lid of your town-issued cart and return it to the DPW yard front office at 629 Randolph Avenue, weekdays between 8:30am - 3:00pm BEFORE June 30, 2025.
Please ensure the interior of your cart is empty, clean and dry before returning, and leave it in the parking area just in front of the main office. If you are unable to transport the cart to 629 Randolph Avenue, call 617-898-4968. If you don't have access to a printer, please hand write the required information and follow the instructions above. Please print clearly. Thanks!
