FY24 - FY25 Rate Sheets

When making insurance enrollment decisions, always consult your healthcare provider and ensure they are within the specific plan’s network or click the "Find a Provider" link on the website for each individual carrier. You may also reach out to the insurance carriers directly, via the Member Services listed on the summary of benefits, to ask specific coverage questions under the plans offered by the Town. Mention that you are a "prospective member" and be ready to reference the specific Group Number as specified below. If you need further assistance or clarification, please reach out to your HR Department and we may be able get further information from our Insurance Account Representative on your behalf.

Staff can log into the Cafeteria Advisors Inc. employee site to check your FSA Balance and File claims 24/7: https://cpaemployee.lh1ondemand.com/Login.aspx?ReturnUrl=%2f

If you want to keep your FSA in FY2025, you must re-enroll. Re-enrollment is not automatic. 

For Fiscal Year 2025, the maximum annual Health Care for the Flexible Spending Account has increased to $3,200.

To change your current Aflac or enroll in any new Aflac, please contact Milton's Aflac representative for a phone call or an in person meeting between before Thursday, May 30.
